How to make a formal complainet

Sorry im posting too much.

But i was wondering how you make a formal complainet to you mental health clinic or the place that you go. Im not sure what else i can do so i want to make a complainet to them tomrrow.

Basically im having trouble with appointments. Ive called them up numerous times, talked face to face, and i keep getting told they have sent me out appointments and i have not been attending. I double checked they had the right adress and they do, but im getting the rest of my mail fine, so i doubt they have even sent them. So then i told them to call me with an appointment so i will deffently get it, guess what? they never did. Now ive been told im discharged for not attending, when i havent even been given a ****ing appointment.

So yeah

Contact your local PALS they should be able to help you make a complaint.

You can also send the complaint direct to the clinic/hospital/trusts manger/head. They have to send it to thier compliants depatemant.

I've had to do this 2, I understand how frustrating and **** it can be. *hugs*
