Is there anyone you can ask? Do you have a CPN or someone who can explain it to you?

I'm not totally sure what it means but would advise asking whoever made your care plan to explain what it exactly means and stuff.
Maybe you could phone up your local social services and ask them to explain what it means. It could possibly be similar to the "at risk" register for children although I am unsure as to what that entails, someone else may know though. I am suprised that you haven't been contacted by social services just for them to tell you that you have been put on the register. I'm known to social services although do not know if I am on any kind of register but I see or speak to my social worker regualry every month.

I think its probably similar to the 'at risk register' young children are put on when there is suspicions in the way they are being treated.
