Care Plan Review

Can someone here explain them to me please? I have one on Thursday and I'm really scared about it (have posted in a more appropriate place about why - I just want some information, please)

Ive got mine tomorrow lol.
There nothing to worry about.
All it is is everyone thats working with you gets together and plans youre care for the next 6 weeks. If you need extra help, who's doing what ect.
Basically planning what help youll be getting.
The usualy start of with a review about how youve been ect by each person who works with you. Theyll find out whats working/not and what would be benifitial.
Youll be asked your opinion ect all the way through.

Thanks. I was worried I'll have to give some input. I'm scared of being in a room with these people and at best I'll be disassociating so not following the conversation well.

Unfortunately there's no-one I could take with me and to be honest, I'm not exactly a trustworthy person right now so don't feel able to ask in case I lose it in the meeting and they see 'me'.

I'm scared enough I'll lose it with the psych people there.

I know this might not be the greatest comfort, but CPN is there for you so I hope he looks out for you like he should do.
