It means that they are responsible for delivering your care, including liaising with your psych, going through your cpa (which is the basis of your treatment. It's a ctp in Wales) and arranging any additional support they feel you need (such as a support worker). They can, with your consent, talk to your family. If they are a nurse (a cc can be an ot, sw or a cpn) and you are on a depot they can administer your medication. They are your point of contact at the cmht. In addition should you require hospital admission they come and visit you at least once a week though in an IP setting, day to day care is administered by on ward nurses with input from your psych.
As for how often you are seen, well it depends on your needs. It can be anything from every week or as often as once a month.
