It would probably depend on the circumstances, but generally unless they think there's some kind of child protection issue (eg you were self harming in front of your kids etc) then it's unlikely they would get social services involved, especially if you are asking for help to stop - most doctors don't like getting social services involved unless they have to. Of course if there's been previous social services involvement it's possible they will be involved again, but if you're trying to do something to better your situation (i.e stopping the self harm) then this will be seen as a positive and responsible move.
