Firstly, just to say: You sound like a really great friend, really caring and understanding.
It's difficult with depression and sometimes there isn't anything that people can say to help, but just saying to her what you've said here - that she's not alone, that you understand and that she can talk about it if she wants to, will be really helpful and reassuring. If you're unsure of how to bring it up, you could always try sending her a text the evening before, or even writing her a little letter or a card to give to her at the end of your day out.
Mentioning your experience with self-harm and depression sounds fine if you feel comfortable with that, and I am sure she will be really glad to just know that she has a friend like you who cares so much.
Look after yourself as well, I know it is difficult when a friend is struggling. x
