discharge from early intervention

Has anyone been discharged from early intervention? I'm coming to the end of my time with them and will be transfered to cmht.
Just wondered what the process was and how different the support is?

How far away from discharge are you?

I'm getting discharged in a few weeks and I have been working over the last few weeks- months on preparing me for discharge. I have my discharge CPA in a week or so where we are going to go through the logistics of discharge. I imagine your team will help you adjust to the process too.

Sorry this post isn't actually answering your questions but wanted to say you're not alone.

Best of luck with your discharge and transfer.

Thanks. I need to be discharged before October but they mentioned it today in my cpa that they are gonna move me quicker so I can make use of the theraphy offered in the cmht (i can currently access it while under early intervention)
Good luck with your discharge CPA x

I've just been dishcarged back to my GP.

I was under the team for over 5 years and we were hoping for me to go to a recovery team for a bit however it was way to complicated so we gave up.

I work in a recovery team and I think there are differences. The care coordinators generally have bigger case loads so I think that would mean appointments being less frequent. When I was in crisis my care co would see me lots and I don't think that would be as feasible.

I think basically a lot would depend on your care coordinator but essentially the EIS work with people for 3 years with the aim that with intensive support people can be discharged back to GP with all the right tools etc. I know there is a push for people to go back to GP's both here and in the borough they tried to get me seen in but this is more to save money as the recovery teams funding is much less per head generally.

That said if you can access therapy that's a huge positive and hopefully you will get a care coordinator who you get on with etc.
