I am currently:
Writing your own care plans

I have been asked to have a go at writing my own care plans. I have some old care plans to look at but I was just wondering what kind of headings to use on each care plan. So far I have:

1) Mental health
2) Physical health
3) Risk management
4) Restraint
5) Nutrition
6) Home leave
7) Aspergers

Can anyone suggest any other headings that I should consider using?

I am currently:

4) Restraint and crisis management? I.e when you are needing emergency PRN or something; it can influence how you cope as to how people respond to incidents, encouraging meds and so on... does that make sense?

Wow, you have your care plans written down on paper? That sounds really useful, I didn't know some teams did that! (My CAMHS only do a plan for the preceeding month: what medication I'm taking, who I'm seeing etc.) If you feel comfortable would you mind sharing what you've written when you're finished? :) You can say no of course!

Restraint and crisis management sounds like a good idea. I'll have to have a think about what to include as I don't have any emergency PRN written up as we are supposed to use our skills instead.

When I've been in the community I don't think they have been written down but when I've been in hospital they have been. Some hospitals have quite vague care plans but here seem to want them to be quite detailed which I suppose is a good idea. I'm not sure about sharing the care plans as they will probably contain a lot of personal information. Sorry.
