It sounds like you're in desperate need of some help. I think you're probably going to be extremely negative about this suggestion, and I'm not sure how it works in the USA, but in the UK we have social workers who can do this thing called a carers assessment where they see what sort of help the main carer needs to look after the person they are looking after and provide professional carers who can come round the house and help with looking after your mother and brothers. I'm sure there must be something similar in the USA? It sounds like your situation is quite complex, with your mother and brothers health needs and your mental health needs as well as being the main carer (which really can put huge amounts of stress on people - as I'm sure you're acutely aware of).
There is no shame in asking for help you know. It can actually be very brave to ask for help. And if it means that your family gets cared for and you can have some more space to just breathe then there's nothing wrong with that and noone would judge you.
I guess you've got to think, if you become too ill, then who is going to look after your family. You need to look after yourself in order to be able to look after others.
If you are point blank unwilling to get some support with caring for your family, then I'm not entirely sure - maybe try doing some CBT work sheets online? Remind yourself it's not going to be this way forever.
Honestly though, it really sounds like you need some time for yourself in order to get your head straight. Would it not even be possible to ask someone else in your family to care for your mum/brothers just for one weekend or even just one day? Just to get a little tiny bit of breathing space for you. You could even just say you are going to visit friends or whatever if you really don't want them to know the truth.
I'm sorry you're going through so much at the moment, it sounds like an extremely difficult situation to be in.
