Writing a corrective statement re: psych records
Ok, I feel like a bit of a dumbass for posting this thread, but nevermind.
I am currently in the process of writing two (maybe three) corrective statements which I would like to have inserted on my psych records. One of which concerns a potential misdiagnosis, the other regards a misinterpretation/disregard of what was at the time extremely distressing levels of suicidal ideation.
I have spoken to the head of my local psych dept regarding this who has given it the go ahead, but I am slightly unsure as to how best to go about it. i.e; should I just send a brief covering letter and attach individual copies of the statements (in this case pesumably both of the individual statements would need to be signed in addition to the letter)? Or would it be better to just write one letter incorporating the statements, if that makes sense?
Also, in terms of actually writing the statements, I have obviously detailed what I believe to be incorrect, and have given my version of events. But I have also briefly included how I feel/felt regarding the apparent discrepancies, and the subsequent action taken by myself as a result - for example feeling disheartened and cancelling future appointments. I haven't written an essay by any means, but I'm just wondering whether that would be considered a little OTT, or sound a little too much like a complaint rather than a clarifying note?! I'm sure it's perfectly appropriate, but it can't hurt to ask for advice.
So by now you will have realised why I feel like a dumbass for posting this, and I apologise if it makes little sense; I really need to go to bed! But I have never asked to have a note attached to my records before now, and don't particularly want to have to bother to do so again. In short, I just want to get it right the first time around, so if anyone has and relevant experience or advice regarding this issue, I would be glad to hear it!
