I've decided to book an emergency doctors appointment. I was ment to be going tomorrow and I just can't cope waiting.
My mental state is horrendous at the moment. I'm self-harming everyday, I cry most the time, I have violent urges and feel angry, I change my feelings every hour. For example last night, I was happy for around an hour then I went to rock bottom and wanted to be dead. I self harmed and banged my head off a wall.
I just wanted to give up. I'm tired 99.9% of the time. I ache all the time.
I'm kind of fed up.
What should I tell my doctor?
I don't want him to section me if I say I feel suicidal all the time.
I couldn't cope if I didn't see my fiance all the time.
Help me.

Just be honest with your doctor, there's nothing more you could do and nothing less that you could do. They don't actually like to section people and your doctor doesn't have the authority without another team, anyway. They usually try to help you find a way through without resorting to hospitalisation. Of course, if you really need that then I'd go with it but they try to work with you within the community first, usually.
Be as open as you can.

Well done on booking the appointment, it's a reallt positive step and you should be proud of yourself.
Perhaps you could bullet point the main things you are struggling with at the moment so that you get everything you wish to say said in the appointments time. Personally I've found this very helpful when I have a lot of sensitive things to discuss (which I find hard to do) and means that the doctor is aware of everything that's going on, even if only a few are discussed in the actual appointment.

It went fine.
I'm on 40mg of Fluexotine(sp?) now and he's referred me to a counsellor.
Also got crisis team for 2 weeks, as I said I feel suicidal every day.
Very emotional appointment.
