Sick Note running out

Ive never been signed off work before and have no idea what really happens in this situation, so advice would be much appreciated...
Ive been off for nearly 2 weeks, but the sick note runs out the day before my next Drs appointment. I dont feel there is any way I can possibly go back to work at the moment. I dont know, obviously, but am hoping the doctor will support me in that, as they suggested time off work last time.
But what do I do about the sick note? Do I have to go into work for the 1 day and then get signed off again?
My work have not been supportive at all, and I am really anxious about contacting them to explain as Im sure they wont be supportive or understanding.
What happens in this situation?
Any advice would be great, thank you.xx

Can you still do the self certify sick note thing? It used to cover you for 5 days I think. Not sure if it's still around tho. Know what you mean about work being unsupportive.

Ring and let the receptionist know the situation and see if you can either get a new one written to pick up or say that you will need an earlier appointment if that is not possible. Don't go back for a day and then get signed off as that can cause problems for absence targets. Are you in a union?
