I am currently:
Mental Health Questionnaire

I finally agreed to go get some councilling and i've been give a questionnaire to fill in before I go, I've filled in the easy stuff like name, nationality ect and even filled in the alcohol section but just cant seem to fill in the rest. Theres questions such as 'briefly describe your current problem and do you feel you have been abused' then you have to tick the right box. Its seems so wrong to tick a box, does that box define my life from now on? and how do you briefly describe something you've lived with for years?

I really dont want to go but I promised I would i'm gonna be a nervous wreck till wednesday, maybe I could just tell them to read my blog instead of trying to squash it all in to tiny sections on the stupid form

If you can't fit what you want to say into the little spaces, you could always add extra info on paper at the end. Or print off parts of your blog and include those (with a note to explain what it is and why you have done this). I know these questionnaires can be really difficult to fill in.

They aren't there to trick you out or get you committed.

The purpose of the form is to give the counsellor a brief idea of what is going on and what they need to help you with the reason it's brief is when they meet you they will expect you to talk about it in more depth.

When I last filled in a form like that I put what I was struggling with down in a bullet point fashion.

Try to fill out what you can - nothing hinges on you completing that questionnaire, there's no right or wrong answers, and you won't be in trouble if you miss a few bits out.

The counsellor I'm seeing now never even asked me for the questionnaire (even though I'd filled it in), some prefer to get those answers in person. Like Random Swirls said, it's just a really useful tool for whoever's treating you as they'll get a brief knowledge of you, and know what does and doesn't need talking about.

Just know that nothing will be decided by your answers. The things that are on there will be explored in detail, so don't worry about it being too little information.
