I feel the same. In my case, it has been diagnosed as dissociation but you would need to speak to a professional to find out what yours is, as it could be different. Also, there are various things that can cause you to feel this way that would be worth checking. Like how is your sleeping, as under-sleeping or over-sleeping can affect it, also certain medications.
I find that I have to try to stay in reality as much as I can, and focus on physical experiences and sensations if I become too disconnected. One exercise I have found useful is to chose a small object, like a stone or a small toy, and hold it in your hand and feel the texture really carefully and concentrate on the physical sensations of it.
It won't cure the problem but it can help in the short-term. but it's definitely worth talking to your doctor/mh professional as it is important to find out whats' wrong and how to deal with it.
