I have been the patient and the friend in this situation.
I was not asked to leave before the doctor asked me any questions although I think my friend was completely comfortable with me there. However you can say that you would rather do the questions alone.
When I was the patient, I was only really asked:
When it started
How long its been
Are there any triggers to each episode
Period stuff
has there been any major life changes
What you want them to do for you
I wish you the best of luck with it :) and I hope you feel better afterwards. I was shitting myself when I went and the second time I went I was alone so it was made worse but then in the longrun I feel more independant. Anyway you may not feel great when you come out as doctors are really all that sympathetic (in my experience) but in the long run you will have done the right thing :)
