Stress and changes in routine can be a major contributory factor in hallucinations (they certainly are in mine) and so it could well be you felt fine in hospital because you had settled into a routine there and had a structure.
Now, out of hospital has changed that routine, you are somewhere else and as said above, are trying to fit back in. Did the hospital suggest you have regular appointments to help with the transition at all, if not, it is worth contacting them to let them know what is happening and ask if there is a way this can be put in place.
That, and setting yourself a fairly fixed routine could help in the mean time. However, if they get worse or you are concerned, it is best to contact the hospital, or your cmht and let them know what is going on. It is much better to let them know than wait and let it potentially worsen (stress, leads to hallucination, which leads to stress etc).
