Health forms

I have just been offered a job which is perfect for me.

Im worried though because i have been given a health questionnaire to fill in and im unsure whether to include that ive got chronic depression and under treatment for it. I really dont want to tell them, its under control and has been for nearly 2 years so its not going to be a problem and even when it was it was never obvious to friends and didnt impact on my work so i dont see why o should include it, plus nobody knows about it only 2 friends and i dont want anyone to ever know.

It also wants info on any medication im on and i dont wantto put that eithet cause im on antiphyscptocs amongst other things and that might make my depression look even worse.

I also have arthiritis which i am going to put on because it affects me daily and im having an operation in a few months for it, so with the depression it might seem like im too much trouble to employ.

The position is only for 3 months and isnt physically demanding and not going to make my depression worse as it is very similar to what i did at uni.

So my question is, is it worth jeperdising a good short time job telling them everything or just mentioning the arthiritis and not depression because its under control?

Thanks

Tell them everything becaus eif they find out you lied you will be in more trouble. They can't take the job off you because you suffered from depression that would be discrimination.

How would they find out i have depression? its never affected my work or anything and no one knows, none of my family and and only a few friends. Can they ask for my health records?

Do i also have to put what medication im on? it asks if i am on any but i dont see what that has to do with anything

It's better to tell them the truth because it enables them to help support you better.

But as has also been said if you lie you may have to be prepared for the potential consequences.

Also yes I do believe they can look into your records or request an assessment if they are concerned for your welfare.

Hope you can figure out what to do.

Thanks for your replies everyone. I am going to put it on because i guess they could find out if they ask about my arthiritis and ask for my gp to write a report on it.

I just panicked so bad because i hate anyone knowing and it has never been a problem as far as work goes so i just dont know why they need to know. I would never ask them for help if anything did happen.

But you are all right and ill put it on but il try and make it more positive like you said dash and say its under control etc.

Thanks!

The information will be kept confidential. Usually it is only given out in a need to know basis

All they are doing is making sure whether you need any reasonable adjustments to do your job. You will probably be called for a meeting with OH and they will request a report from your GP. As long as you are fit to work, or can work with adjustments, there it isn't anything to worry about.
