Hey :) I agree that writing stuff down can help, it's often a lot easier than saying how you're feeling out loud, I know that can be a challenge. I definitely understand that you're nervous to go to the doctor and tell them what's been going on, but you shouldn't feel humiliated or ashamed to admit that you think you may have a problem. Doctors see a lot of people who are asking for help, and trying to figure out why they're feeling the way that they do. It's the doctor's job to give you answers, and no one is judging you or seeing you any differently as they already do if you tell the doctor that you feel as if you may have a problem with your mental health. It's a scary thing at first, but when it's over with it's actually no big deal at all, and you may even end up feeling better knowing what's been going on to cause the way that you have been feeling.
