can anyone help with my collge work? about horses

Hi
Ive got to put this survey out about lamintis i was wondering if anybody on here could complete it please?

Have you ever had a horse/pony that has got/had laminitis? Yes/No

If so....

What breed was it?

Rougly what condition score did it have at the time? (if you don't know about condition scoring PM me and il send a copy but ill try and post one later)

How was your hors kept? E.g. living in, living out, living in and out

What type of work ws your horse doing? e.g. dressage
Showjumping
Cross country
Flat schooling
Driving
Out of work

Roughly how many hours a week was your horse doing at the time?

How was your hors kept? E.g. living in, living out, living in and out Out for summer/In at autumn

What type of work ws your horse doing? e.g. dressage
Showjumping
Cross country
Flat schooling
Driving
Out of work Out of work or random small hacks/show jumping

Roughly how many hours a week was your horse doing at the time? 2 or less/7 to 9

I have definatley done it right, 1st is a cushings pony though then after / is an older pony with suspected cushings but it was not.

How was your hors kept? E.g. living in, living out, living in and out Just been turned out for summer, two weeks previous to in on a night and out on the day.

What type of work ws your horse doing? e.g. dressage
Showjumping
Cross country
Flat schooling
Driving
Out of work Flat schooling and hacking

Roughly how many hours a week was your horse doing at the time?Only about 3-4

I am currently:

Not mine but one of the ones where I used to work and still ride

Welsh Sec A
Condition score 3-4 (closer to 4)
Lives in but has a couple of days in the field each week
Riding school pony, mainly flat work (not strenuous work - mainly walk and trot) but also a little bit of jumping
On average probably about 10 hours work a week.

What breed was it? we arent really sure, but some kind of welsh pony we think, she is old so didnt have a passports when we got her 10 years ago lol.
