Try not to worry about it. What they meant by confidential was probably that only the person conducting the interviews would have read them, no-one else. They should have explained that you'd be having interviews because of it though as no-one should have to do a survey that haven't been properly informed about it. They really should have explained beforehand that peopple will know what you've written because in that situation, I would've freaked out!
They will only be doing this so they can try and help you. They'll have noticed that there may be some problems within the students and just wanted to see if they can identify them and try and see if they can resolve them before they get worse. Just try and stay calm, it's probably another way of getting you help, even if it's getting you help by changing how the school is run so they can make it less stressful. They won't be wanting to punish you or anything as they have no reason to. At my uni we had to all do a survey (which was completely confidential, no one found out who'd written what) about EDs/Depression because the student union realised how much of a problem they were around college. They took the results and now, as a result, we have counsellors come in often to help us deal with stuff so it is probably for stuff like that.
Try not to harm yourself, I realise how hard this must be for you but harming yourself won't make anything change and you'll still start worrying again about it. It would be more productive to go and privately talk to the teacher that made you do the test and say that you're worried about what will happen to the results and see ifyou can have more information about it. You don't have to say why it's bothering you. Harming yourself is a quick fix but afterwards it you will probably feel guilty because of it and you might end up feeling as if you are in a worse state than you are now because ultimately it won't have changed a thing except putting more cuts/scratches/burns on you which will just remind you of the problem everytime you see them.
