In my opinion, it is better to discuss this with them now and let them know you are sorry for not saying it at the time, you felt eager and as having fixed appointments is fairly new, it did not cross your mind.
Work-places, however big or small have a duty of care to their staff - whether on a flexible contract or not, and I am sure they will be able to find a way to make your hours work for you so that you can do both. There is no reason not to.
Once you have a little more faith in the managers there, and if you are looking to stay it may be wise to pull one aside and go into more detail about your mh, purely so that they are aware and have the opportunity to provide or allow for any support you may need going forward.
