Mental health Survey for school *your imput would be very helpful :}*

Hey Everyone,
for one of my classes we are doing "the future of ASE" as our Independant Study Unit. I decided to do mental health. But I want to make sure it's okay. I've gotten a couple back from people, but they were all from people who don't have a mental health problem. So, could you guys look it over to make sure it's not insulting or anything and just to give me your opinion. You don't have to do it!! I'm handing it out at my school, so I'm not looking for answers to the survey. The Crew's room is the student lounge, that's what they call it. And ASE1 is just the name of the school. Thanks for your time guys!

Mental Health At ASE 1

At ASE 1, there are many different languages spoken and countries represented. What many of us don’t realize though is that many students here are facing mental health issues. By participating in this survey, you’ll be helping to better understand how students at ASE cope with mental health issues. Participation in this survey is voluntary and anonymous. You may skip any questions on the survey that you do not feel comfortable with answering. If you have a different answer to any question, or want to add to your answer, feel free to write in the margins. Once you have completed the survey, please place it in my mail box or return it to me as soon as possible, but before May 23rd 2008. Thank you very much for taking the time to complete this survey.
Emily

Dramatic

Personally? I wouldn't find any of it offensive.
I can see some of the questions being quite personal (i.e. about sexuality/gender) but i think it should be fine as those who are uncomfortable in answering such questions can skip - as you've given that as an option.
Goodluck :)

I think question 22 is the only vaguely 'offensive' one there.....perhaps you could re-word it and change "safe" to "comfortable" or something?
And counsellor is spelt counsellor not councillor (Y)

In the list of diagnoses, you missed out depression. And Bipolar.

I'm not sure of question 22, either. Maybe you could put "Do you think people with mental health diagnsoses should go to separate schools that can help them?" Or something like that?

Just some technical points take them if you want. The age I think should be left as a space for them tow wroite as the circling looks messy. Same with the gender question, it seems a bit leading to write trans gender as people who are transgendered I would expect would prefer to be seen as a women/man (opposite to their sex). Same with sexual orientation as you may have curious people especially in a school. For question 9 I'd put tick boxes next to the disorders to make it clearer for participants. 11b leave open for the participant to answer or tick boxes. 13. Just wondering why you need to say who? people may be reluctant to reply on this basis . 17 sounds a bit odd could use a bit of rephrasing to make it sound politer it kind of sounds like they shouldn't feeel accepted because of their mental health. Question 19 is the same at question 15 pretty much. If you werent aware of the problem then you wouldnt know they had mental health problems in the first place. 22 seems bit offensive dont know if you could reword it .Question 21. For the scale use simple phrases such as Definately, Usually, Not at all.
