if it is anything like the assessment I have had for looking at your needs, then I was asked questions about all areas of my life, MH, physical health, culture and religion, social life, relationships, psychological support, practical support, any risk factors from myself or others
As a result of it I have been allocated a support worker to help me to get out and about and attend some social things and have more of a routine in my life. It also helped my CC to understand my issues and difficulties more.
Obviously yours may be something completely different, but that was my experience of an assessment to look at my needs. sorry if it is completely different and therefore this is a completely unhelpful answer.
