I am currently:

Lower and lower
I had been diagnosed with cyclothalmia a few years ago and as i was also having problems with my medication, I was told by my cpn that i should stop the meds and try to live my life as best as possible. That was 2008 and i am now further down than before, no sleep, energy and generally cant be bothered. Argued with everyone, world seams against me. I dont want to go back to the dr as i know they will start me on another course of drugs which will cause me to think badly about cutting myself. If i could just stop the repaid thoughts and lift myself a bit

I think speaking with your doctor about this is a good idea, you don't have to go on mendication if you don't want to but you can discuss what's going on with you, how you're feeling right now and perhaps speak about your experience with medication and work out an efficient plan with your doctor as to how to approach and deal with what's going on.
Medication is a difficult thing, most of the time it is a trial and error process which can become very frustrating. However, when you do find a medication that works for you it can help lift you out of the deep darkness enough to work on getting yourself better. Try and remember that.

I think you do need to let your Dr know how much you are struggling right now. I will in some ways, echo what has already been said. Not all medication will have the same affect and it may be that there are medication options out there that are more suited to you.
It is up to you whether or not you decide to take medicines, do know though that there is also therapy, counselling and other support out there. Most importantly, someone knowing you are struggling.
Do please keep reaching out for help, it sounds like you do need it right now.
