So it's been properly open for a year?
It could be that bacteria got into the wound as it wasn't fully healed and it became infected and it has caused an abscess under the scar. The is backed up by it being hot/swollen/painful.
It could be something else though like a cyst
It is probably best to get it looked at especially if the original wound didn't heal it might need re-opening, the scar tissue cut away and be sutured up to ensure it fully heals. However, for a wound not to heal after a year is VERY bizarre.
Do you have an ED?

As mentioned above, you need to get this checked out. You might just need it to be closed properly to prevent further problems.

If you have an ED you might be deficient in some of the vits and mins that are required for healing such as Zinc and Vits a and e that come to mind but still after a year it should have healed unless you've kept re-opening it or something.
It may just be that you've produced excess scar tissue but I'd have thought that would have occurred by now unless this is just the whole of the wound finally closing. Cysts can be quite hard or as you say its swollen/ painful it could be an infection if it definately hasnt healed fully (not that I'm saying I think you're lying but it could be that it looks it from where it is like it hadnt but it might just have a scar that is lower at that point). I'd go and see a doctor as it is painful and you're not sure what it is.
