There was a very interesting whistle blower report done for the BBC some time ago from an OFSTED inspector:
Is this the program where a woman worked in nurserys that had had a bad report about them?
If so, one of the nurserys on there (where a little girl died from choking on an apple - she was too young to be given a chunk of apple to start with) which was closed down, happened to be one of the nurserys that offered me a placement to work (it was in a place in Cambridgeshire called Cambourne) which i declined, as i had a friend that worked there who left after 3 months as it was so poorly run!
Out of 8-9 nurserys i've worked in, i'd say i'd only put my child into 4 of them..
