Don't worry about it. Even if you go to the doc you don't need to tell UCAS, so don't let the fear of that put you off going to the doc to get help. I filled out mine a few months ago but don't remember anything about mental health. I know for nursing I had to cuz it's not through UCAS up here, its through a company called CATCH, and after the occupational health, when they were satisfied it was controlled (hidden, not controlled atch) then they were happy enough.
