It seems like your anxiety is impacting on your life in a big way. I know when anxious is hard not to think about what others think of you however you could seek help and therapy without anyone knowing. Perhaps therapy privately would be a good thing for you to consider as others wouldn't know so wouldn't be able to judge you or form an opinion on your situation?
Going to the doctor is of course a scary experience. Do you have a family member or a close friend who you can trust to come with you for some moral support? Perhaps someone who wouldn't leek any information to others after the appointment. Going to the doctors is a very positive step and sometimes you have to put yourself in that uncomfortable position to open many doors for yourself. Take care,
