Not in the US they haven't. We're not allowed to smoke in public buildings, or with a certain distance from the doors, windows, or air intakes of buildings, but people can and do smoke outside in public areas. I can't walk to class without passing through a cloud of smoke at least once, and I can rarely walk on the sidewalk anywhere without having smoke blown in my face every few minutes. And I'm not even going to start about people who smoke in a crowded place where I can't even get away from them if I try (and really, I shouldn't have to constantly go out of my way to avoid having to breath large amounts of cigarette smoke even when it is theoretically possible to do so).
