Apparently in Colorado, poll workers gave 20 voters a plastic stylus instead of a pen to mark their paper ballots. The voters complained, and were told not to worry - it was invisible ink. The poll workers then tried to feed the blank ballots into the voting machine, which rejected them (as it was supposed to), and the poll workers then tried to manually override the machine's rejection of the ballots. Elections officials investigated, and concluded "there does not appear to have been any malicious intent, just shear stupidity."
