I have one, which details medication that I wont take (for example mine is risperidone due to the fact I had a bad reaction to it in the past which was prescribed under a different psych and also flupentixol because of the side effects were unacceptable). I am also highly against ECT, so mine has that I do not wish to have ECT under any circumstances.
I have also put that I only want certain people to deliver my depot (two specific nurses in the cmht including my cpn because simply I am terrified of needles and I know I wouldnt trust the nurses there) and also if I am agitated I would only want female members of staff to talk to me (I have SA issues so I find it easier to talk to female members of staff).
Mine also covers who would take care of my pets (I have 3 cats), and it says who their vet is and the medical condition they all have. It also covers children but I do not have any so I put n/a in the relevant section.
I need to update mine as I wish to be given diazapam before any blood tests because I am scared of needles (if I were going to a blood test in an outpatients setting that is what I would do). I have asked for diazapam before when I was last IP but they refused, which made me even more anxious.
