I would say that a member of my family is very unwell and I need to spend more time helping them. If they ask more just say you find it too hard to talk about. People are unlikely to question beyond this point.

You do not have to tell them you are ill. It is none of their business.

I would not normally condone lying but since it is a voluntary organisation the only people you are obliged to be truthful to is occupational health and they are not allowed to tell your boss or colleagues. Unless you work in a hospital, or even if you did, I doubt that you would be sent to occy health for postponing a voluntary position.
Don't worry you will be fine.
