I just thought I'd chime in on a point made earlier about the tone of the original post: I don't agree that a first warning (or any warning) should necessarily be gentle and coddling, as the point of a warning is to make it perfectly clear that a certain behaviour is unacceptable and will be punished if it continues, and in my experience, people will respond better to a warning if it's firm and authoritative than if it comes across as a whiny, submissive plea (as has been alluded to earlier in the thread).
In short, a warning needs to be harsh or it will probably be ignored, so long as it's not rude (and the original post did not strike me as being particularly rude).
