I think that's the same as an advance statement in Scotland. Is it where you write how you want to be treated or don't want to be treated if there comes a point where you are admitted to hospital against your will? On mine I just wrote that I didn't want to be given a certain medication because it makes me hallucinate, i'd rather not be given sedating medication, and ECT should be used as a last resort. I hope that helps.
