I am glad you are going to get an appointment - it is an incredibly brave thing to do. It is also the right thing to do, you need support right now as you can see that things can not continue the way they are and your GP should be able to offer you some of that support.

I think discussing things with your manager was a good thing, it sounds like they are concerned for your well being and so are looking at what they can do/advise to help you. Going to the Dr will help with this and your work may ask you to speak to an Occupational Health advisor, who will look at what work can do to help you (rest breaks, counselling, less hours if needed) so that they can be sure you are safe to be in work.
