Well done on booking the appointment, it's a reallt positive step and you should be proud of yourself.
Perhaps you could bullet point the main things you are struggling with at the moment so that you get everything you wish to say said in the appointments time. Personally I've found this very helpful when I have a lot of sensitive things to discuss (which I find hard to do) and means that the doctor is aware of everything that's going on, even if only a few are discussed in the actual appointment.
