What joint problems do you have? I used to have Juvenile Rheumatoid Arthritis but i'm (thankfully) in remission at the minute.
I think you should keep trying to bring this up until you get your point across, if it doesnt work by being subtle then just be direct - he's your boyfriend he wants to be there for you. You say he was brought up to not talk about feelings but that was his childhood not yours, just because he's like that doesnt mean you need to be too. You need to talk about this, you want to quit and you need help, your boyfriend is the best person to offer your support!:) once he realises you need and want help im sure he'll be there for you.
