How d'you get triggered for no reason?
I'm guessing that pretty much there is something that stirs up things each time. Which might be rather hard and painful to look at, but is worth doing so, rather than dismissing. That way, you slow your mind down, and hopefully are less likely to hurt yourself as a result of whatever's going through your mind.
Does that make sense?

What do you really need?
