I think in the ED board it depends on your own opinion really which is often scewed. I think a minor reference to how you are but is negative should be marked as mildly triggering whereas those that go into more detail or could be seen to go into a lot of emotional stuff that can really affect a lot of people in there (but is not against the rules, if it is, don't post it ;) ) may be more suited to having a severe one.

(Just to let people know who are reading this; as said in the forum description, the First Aid Forum is one where things are going to be triggering and labels do not have to be put up for this as it should be assumed that things on there are/ may be triggering though the amount that this is seen should be down to the rules and sorted by the mods/ FAAs. For us in there it is also MUCH more useful if you use the labels in a way that describes the question so don't just use the SI one because you're wanting info on wound care regarding an SI wound; the wound care one is ther fore that reason. These labels are very important to the order that the team members look at the threads and also potentially how they're dealt with so there is a reason for the labels in there and why there isn't a triggering one. If anyone wants further clarification on the labels in there just ask.)
