OK honey.. you seem really nervous and worried but you shouldn't be as this is something you can fix! :) so have faith/hope!
If you need to change your social worker for what ever reason then first off tell/ask your GP.. then go to the Social Work Department and tell them.. or you can look their phone number up and phone them :) They will be nice and understanding.
Can I just say - its so wonderful for you to recognize that you need to chance your SW in order to move on etc! Very brave and smart of you.
May I ask why you don't see your previous Social worker now? is there any chance you can get her back? Why would it not happen?
Phone your old social worker up and ask her if she can come back and help you :)
