Kirsty I think this is a really good honest letter that says just what the Doc needs to know.
I know you are freaked out by the idea but you just need to go through with handing over the letter and take it from there.
Maybe reminding yourself that in 2 hours time the appointment will be over and todays difficult task done will help?
