They will already have some information on you as your GP will have included reasons for the referral.

But I would imagine in the assessment they will want to hear from you what your struggles are. They will probably ask lots of questions to get a better idea of how they can help you. They wont expect you just to talk, they will guide you through what they want to know (and if they ask you something you aren't comfortable talking about, just tell them that).

Don't worry about it, it's not a mistake. At least have the assessment, then you can voice any concerns you might have with them and they can talk things through with you.
