I have seen a lot of posts over the years asking about what sort of questions your asked on a first meeting with the primary mental health care team (UK), to give you an idea I thought I would post up these images for your reference. This is what I was sent in the post and asked to complete it prior my meeting and take it with me.

I must add that these are in no way to be reproduced or used for anything but educational purposes and are not designed to help you self diagnose, they are purely an example of what you MAY be asked at an initial assessment.

I Hope this is useful to people and moderators, if you feel this is a good post could it be stickied?

if i was given something like this before an assessment i dont think i would have run into so many problems. im assuming different areas do different things, like here we appear to not get questionnaire thingys

thanks for posting this. i recognise the first two, i think i've only ever filled them out in a therapeutic community though, not as part of CMHT. but i've never seen the second two.

i remember filling one form in that asked the question: "true or false - i feel sexier than most people". i so wanted to put a big, thick circle around "true".

LOL, I wish these ones had amusing questions instead of the usual nhs crap!

Ever thought about hurting yourself, lie and it's yourself your cheating, tell the truth they want to section you. Oh fun fun times

I had one with a million statements that I had to circle true or false, one question was "I have been on the front cover of a magazine this year". Made me laugh then I wondered what happens if a celebrity ever needed to fill out the questionnaire.

I just had to fill in a 186 question personality questionnaire for my psychologist and those things are tedious to fill out.
