When I go into chat I see a blue screen and `Authentication Failed`.
Does your username use any special characters?
Chat is old and it was written in the days before all Unicode Characters (special characters) were supported in usernames. There are known issues with <, >, ~ and *. Unfortunately the only way to fix this issue is to change your username to use letters, numbers, spaces, minuses, underscores, full stops, and square brackets. There is a possibility that other characters may work but I can't guarantee it.
What time is it?
For an unknown reason, chat throws this error for everyone between 00:00 and 01:00 UK time [check the time]. The only way to solve this issue is to enter chat when it is not in that hour window.

As I said above chat is an old flash-based program. Therefore is can be particular about the combination of browser and flash version.
Are you running an advert block or script block extension to your browser?
Anti-script extensions block chat from running. Turn them off and restart your browser.
Try clearing your cookies and other temporary files.
For most browsers, do the same actions that you did to clear your cache but add cookies and temporary internet files to the list of things to be deleted.
Are you running the newest version of your browser?
Try updating your browser. Chrome, Firefox, Safari, and Opera.

Does this problem also happen in other browsers?
Internet Explorer has never had a great track record of being able to use chat and most other browsers that aren't listed above (GNU IceCat, RockMelt, SRWare Iron, Mosaic, MSN, etc) are either too old, too obscure, or not maintained enough to use with chat. If you are in a bind, Chrome seems to have the best luck with supporting chat.

Due to a falling out between Apple and Adobe, flash only kinda works on IOS devices. People in the past have had decent luck with Puffin which has flash built in.
None of this helped.
To make sure that someone can help you, please provide the issue you are having as well as your browser name, browser version, flash version, antivirus type, operating system and what you have already tried on your own to fix this problem. There is a good chance that someone will help you in 24-48 hours.

There's also a glitch between midnight and 1am BST. You won't be able to enter chat due to authentication failure but if you are already in chat before midnight it won't kick you.
