pip and si

I recently received a letter from the dwp saying that my dla is ending and I have to claim pip.

Has anyone ever put that they SI on their pip form? Although I have not SIed for several years I don't feel fully recovered from it and that one day I will still si.

As it's something that effects my mh I feel as though I should mention it on my pip form. I also read disability rights UK guide to pip and it specifically mentions si.

I haven't gone through the PIP process yet but I put it on my DLA form. I think SI and ODs etc are relevant to put on the forms.

Yes, I put SH on mine as it reflects the mental illnesses with which I struggle.

I mentioned it on my PIP application.

If you haven't seen this PDF before I really really recommend it: http://www.lampdirect.org.uk/sites/d...sment_form.pdf

It gives examples of some of the indicators related to mental health for each question, and it's a really useful prompt for helping you to describe your difficulties - there are a lot of things I hadn't really thought of which can be relevant. Things like feeling so low that you don't cook or eat properly, thoughts of hurting yourself around mealtimes, need for supervision or support with medication due to risk of self-harm or previous ODs, all sorts. I really really recommend having a look through it.

Thanks Too Shy. I got my pip form today and I am going to attempt to fill it in whilst waiting for welfare rights to help me. That link is certainly useful.

Ok I have looked at the form and I have written down my responses on a spare piece of paper. However I am confused where to put down that I SI. Do I put it on the bit about managing treatments bit or the additional information part?

I put it in the additional information bit, but I don't know if I was right to do that. It should be something that Welfare Rights can advise you on.

Yes some bits I am confused on hence why I need the support of welfare rights. Hence why I put it on a spare piece of paper.

I also suffer as part of my mh a vinegary taste which effects my ability to eat (even chocolate tastes vinegary) again I am confused where to put it.
